2007 - 2008 APPLICATION

PURPOSE:
The Golden Apple Teacher Recognition Program honors outstanding teachers, who upon induction into the Golden Apple Teacher Academy, will:
  • Promote an awareness of educational excellence in the region and encourage a partnership between schools and community,
  • Recognize and support best practices in teaching, and
  • Promote learning as a lifelong process.
ELIGIBILITY-(Must Meet All Criteria):
  • Nominated, certified, 6th -12th grade teachers in 06-07 and Pre-K-5th grade teachers in 07-08
  • Teachers with five years of full-time teaching experience
  • Currently teaching full-time in a public school or ISBE recognized non-public school in Winnebago or Boone County
AWARDS PROCESS:
  • Complete applications and submit to the Golden Apple Foundation office by 5:00 PM on Friday, December 1, 2006
  • Selection committee reviews applications (with applicant identifying information omitted) and selects 20 semi-finalists for classroom observations by December 15, 2006
  • Trained community volunteers observe and evaluate 20 semi-finalist teachers in their classrooms during January 16 -February 22, 2007
  • Interviews conducted of 20 semi-finalists on Saturday, February 17, 2007.
  • Golden Apples presented to five Teachers in surprise classroom visits on March 9, 2007.
  • Golden Apple Recipients honored at the Eleventh Annual Golden Apple Teacher Recognition Banquet at Ramada Plaza Suites in April.
AWARDS FOR 5 SELECTED GOLDEN APPLE RECIPIENTS:
  • Inscribed Golden Apple
  • Membership in the Golden Apple Teacher Academy
  • Tickets to the Golden Apple Banquet
  • Recognition at the Golden Apple Awards Banquet
  • Cash award
  • Professional development stipend
  • Rockford College scholarship
  • Educational software to the teacher's school
  • Highlighted in 23WIFR special program on excellent teachers
GUIDELINES:
  • Please type or print clearly. If using a computer or word processor, you may use your own paper, but only use the amount of space and size of paper provided on the application.
  • Fill out both sides of the application. Answer all questions.

Please type or print the following information and include it with the application:
  • Using the available schedule grid, a detailed copy of your schedule for the January 16- February 22, 2007 observation period. Please include lunch period, break times, and class beginning and ending times.
  • An explanation of any special events or conflicts that can be predicted in your schedule during the observation period.
  • An explanation of any special situations that exist in your classroom that would assist observers in their understanding of your teaching responsibility. This may include special students, inclusion students, team-teaching situations, or the use of parent volunteers.
LETTERS OF RECOMMENDATION:
Please provide three letters of recommendation, with at least one of the references from an educator who is familiar with your classroom teaching. Each person sending a recommendation must include in the letter his/her name, address and the relationship to the applicant. These letters must be included with your application.

DEADLINE:
Application and three letters must be received by Friday, December 1, 2006 at 5:00 pm.
